Job summary

The Centers Plan for Healthy Living/Centers Care Solutions is seeking a Care Coordinator to join a multidisciplinary team serving members in the Managed Long Term Care program. You will help schedule and arrange treatments, coordinate communication among field assessors, care managers, and physicians, and ensure accurate documentation of visits and outcomes.

Ideal candidates have a HS Diploma, strong customer service and communication skills, and proficiency with Microsoft Office.

Qualifications

• HS Diploma required; 2 years customer service experience; strong written and verbal communication skills; computer proficiency.

Responsibilities

• Assist in scheduling ongoing member visits (e.g., UAS Assessments) and track/reschedule appointments.
• Meet daily scheduling quotas.
• Provide daily report of all scheduled visits.
• Upload UAS documents into member files.
• Generate member correspondence and handle mailings as needed.
• Coordinate communication between Field Assessors and the care team.
• Document daily calls and care coordination activities.
