Job summary

Vanderbilt University Medical Center in Nashville, TN is seeking a regulated RN Case Manager to coordinate patient and family interventions across the care continuum. You will remove barriers, promote efficient resource use, and serve as an advocate for patients and families.

The role supports Pillar outcomes and system improvements, working within the Transition Management Office. BSN or Master's preferred and ADN with experience may be considered.

Qualifications

• RN licensure required in the United States.
• Experience in care coordination and case management.
• BSN preferred or higher education.

Responsibilities

• Assess patients with complex clinical needs for coordinated care.
• Develop case management plans to achieve patient/family outcomes.
• Monitor progress and remove barriers using improvement methodologies.
• Incorporate ethical considerations into practice.
• Provide education and guidance to the interdisciplinary team on complex care.
