Job summary

Vanderbilt Health is seeking a Nurse Coordinator to collaborate with the health care team to coordinate patient and family care across the continuum. The role emphasizes removing barriers and promoting efficient resources, while supporting clinical quality and pillar goals.

Responsibilities

• Assesses and identifies patients with complex clinical needs that require a coordinated plan of care.
• Develops case management plan to attain optimal patient and family specific outcomes.
• Analyzes progress on the plan of care and removes barriers using process improvement methods.
• Integrates ethical provisions in all areas of the practice.
• Demonstrates leadership and serves as information resource for the interdisciplinary team.
• Additional duties may be assigned.
