Job summary

You’ll serve as an advocate for patients and families, contributing to Pillar outcomes and system improvements in finance, satisfaction, and clinical quality.

Qualifications

• RN licensure required with current license in good standing.
• 3 years of relevant work experience.
• Bachelor's degree in Nursing required.

Responsibilities

• Collaborates with the health care team to coordinate patient and family interventions.
• Develops case management plans and analyzes progress to optimize outcomes.
• Advocates for patients and families to ensure safe, efficient care.
