Job summary

Vanderbilt University Medical Center is seeking a skilled nurse to lead case management and care transitions within the Care Transition Center. You will coordinate complex post-acute care, collaborate with clinical teams, and connect patients and families to needed resources and follow-up care.

The role emphasizes effective communication with providers, facilities, and community partners, ensuring smooth handoffs and timely access to services.

Qualifications

• RN licensure required.
• Bachelor’s in Nursing and Master’s in Social Work preferred where applicable.
• Experience in post-acute care and care transitions is highly desirable.

Responsibilities

• Leads care coordination across the care continuum.
• Manages post-acute care workflows and transitions to appropriate levels of care.
• Collaborates with families, providers, and facilities to meet patient goals.
• Establishes outpatient follow-ups and ongoing communication with ambulatory providers.
• Oversees post-acute hotline triage and resource deployment as needed.
• Serves as liaison with legal conservators in care decisions.
• Acts as primary customer service representation for post-acute services.
