Job summary

Qualifications

• RN with a valid license and 5 years of work experience.
• ADN + 5 YOE required; BSN or Master’s preferred.

Responsibilities

• Assess and identify patients with complex clinical needs requiring a coordinated plan.
• Develop case management plans to attain patient and family outcomes.
• Analyze progress and intervene to remove barriers and improve care.
• Integrate ethical provisions in all areas of practice.
• Lead as a knowledge resource for the interdisciplinary team on care coordination.
