Vanderbilt Health seeks an experienced case manager to lead patient care coordination across the continuum. You will collaborate with clinical teams, facilities, families, and community resources to address barriers and optimize follow-up care.

The role emphasizes psychosocial support, efficient transition planning, and active participation in maintaining high-quality patient outcomes within a renowned academic health system.

Formación

• Experience in case management and care coordination in healthcare settings.
• Strong communication and interpersonal skills with patients, families, and teams.
• Ability to navigate psychosocial issues and healthcare financing options.

Responsabilidades

• Leads patient progress through care continuum with outcomes-based practice.
• Manages post-acute care communications and triages calls to deploy clinical resources.
• Transitions patients to appropriate level of care and coordinates follow-up appointments.
• Maintains outreach and satisfaction contacts with network providers and families.
• Collaborates with legal conservators on patient plans of care.
• Provides leadership to Case Managers in coordinating patient care.
