Qualifikationen

• Graduate from Specialty Training Program-Nursing (Required)
• Bachelor's Degree-Nursing (Preferred)
• Minimum of 2 years-Nursing (Required)
• Basic Life Support Certification (BLS)
• Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania

Aufgaben

• Provide primary clinical support to the provider, patient and family.
• Assist with triage of inbasket messages and management of patient interactions.
• Educate patients and families on care coordination and disease management.
• Coordinate laboratory and diagnostic tests for disease management programs.
• Administer medications and vaccines under provider direction.
• Maintain documentation in EPIC for disease management activities.
• Collaborate with primary care providers to follow evidence-based guidelines.
