Job summary

Geisinger is seeking a part-time RN to join an interdisciplinary team. The role focuses on preventative care, triage support, and care coordination for patients and families. 20 hours per week, 8:00 a.m. to 4:30 p.m., Monday through Friday, with responsibilities across Med/Surg clinical care.

Qualifications

• At least two years of RN work experience is required.
• Previous Med/Surg hospital RN experience is helpful and preferred for success in the role.

Responsibilities

• Provide primary clinical support to the provider, patient and family.
• Assist in triage of inbasket messages and management of patient interaction with the clinic.
• Coordinate disease management services and develop individualized care plans.
• Educate patients and families on care coordination and disease management principles.
• Collaborate with primary care providers to ensure guideline adherence and follow-up.
