Qualifications

• At least two years of RN work experience is required.
• Previous Med/Surg hospital RN experience is helpful and preferred for success in the role.

Responsibilities

• Promote preventative patient care as part of an interdisciplinary team.
• Coordinate disease management services and provide primary clinical support to patients and families.
• Assist with triage of inbasket messaging and manage patient interactions with the clinic.
