Geisinger is seeking a Nursing Navigator to assist patients entering care programs and coordinate comprehensive treatment plans. You will educate patients, liaise with caregivers, and support pre- and post-hospital care within a collaborative team.

We value experience in patient education, care coordination, and interdisciplinary teamwork. This full-time role involves 40 hours weekly in a clinical setting with potential for growth and ongoing professional development.

Formación

• Must have 2+ years related nursing/clinical experience.
• RN license or equivalent per state requirements.
• Experience coordinating care across teams and settings.
• Ability to educate patients and families effectively.

Responsabilidades

• Facilitates entrance of new patients into care programs.
• Provides patient education and promotes understanding of conditions.
• Coordinate pre- and post-hospital care.
• Serve as liaison between patient and caregivers.
• Coordinate multidisciplinary clinics and patient activities.
• Provide clinical support to providers and patients.
• Collaborate with other healthcare providers on care plans.
• Maintain follow-up and surveillance for post-visit patients.
• Educate team members in area of expertise.
