Job summary

UMass Memorial Health in Massachusetts is seeking an experienced Registered Nurse for discharge planning and case management. This full-time role focuses on coordinating patient care, assessments and creating effective care plans.

You will work with physicians and care teams, navigate third-party payer processes, ensure timely communication and quality improvement participation while meeting regulatory standards.

Qualifications

• Massachusetts licensure as a registered nurse.
• Three (3) years clinical experience in discharge planning or case management.
• Ability to effectively use specialized computer-based systems for the gathering, reporting, and analysis of clinical data.
• Excellent organizational and communications skills.
• Preferred: Utilization management and third-party payer knowledge.

Responsibilities

• Performs assessment of patient's needs and develops plans of care with input from patient/family.
• Performs admission and concurrent reviews; discusses cases with third-party payers and educates practitioners on care requirements.
• Collaboratively facilitates care across the continuum and coordinates with Care Coordination Team.
• Works with patients/families to establish objectives and action plans within the indicated level of care.
• Contacts third-party payer to clarify benefits and available resources; researches non-reimbursed services.
• Coordinates, implements, monitors and evaluates discharge plan outcomes; revises goals as needed.
• Continually evaluates care level; educates patient and team on resource options and decision making.
• Participates in system-level quality improvement and adherence to clinical guidelines.
• Fosters compliance with regulatory requirements and professional standards.
