Job summary

UMass Memorial Health in Worcester, MA is seeking a Registered Nurse for discharge planning and care coordination. You will assess patient needs, develop care plans, engage resources, and support timely discharges.

Requires MA RN licensure and 3 years in discharge planning or case management; proficiency with case management systems and strong organizational/communication skills. Sign-on bonus may be available; the role is non-exempt, 40 hours weekly, with day shifts and weekend/holiday coverage

Qualifications

• Massachusetts licensure as a registered nurse.
• Three (3) years clinical experience in discharge planning or case management; CM experience waived if applicant completed relevant program.
• Ability to use specialized computer-based systems for data gathering and analysis; strong organizational and communications skills.
• Utilization management and third-party payer knowledge is preferred.

Responsibilities

• Assess patient needs and develop plans of care with appropriate resources.
• Coordinate and monitor discharge planning processes and outcomes.
• Collaborate with care teams, physicians and payers to ensure timely, appropriate care.
• Educate patients and families and document care plans and progress.
• Identify opportunities for improvement and participate in quality initiatives.
• Ensure compliance with regulatory requirements and hospital standards.
