Job summary

UMass Memorial Health in Massachusetts is seeking an experienced Registered Nurse to lead discharge planning. You will assess patients’ needs, coordinate care with the team, and ensure appropriate resources are arranged for a safe transition from hospital to home.

The role requires 3 years of discharge planning or case management experience, MA RN licensure, and strong data-collection and communication skills.

Qualifications

• Massachusetts RN license required.
• 3 years clinical discharge planning or case management experience.
• Proficient with computer-based clinical data systems.
• Excellent organizational and communication skills.
• Preferred knowledge of utilization management and third-party payers.

Responsibilities

• Assess patients' physical, functional, and psycho-social needs.
• Perform admission and concurrent reviews as assigned.
• Coordinate discharge planning with care teams and payers.
• Educate patients and families on care plans and resources.
• Participate in quality improvement activities.
