Job summary

Qualifications

• Associates Degree in Nursing or Diploma in Nursing; Bachelor's preferred.
• RN license active in Georgia; BLS required.
• Minimum 1 year nursing experience in the acute care setting.

Responsibilities

• Assess transitional care needs and psychosocial factors; plan discharge.
• Coordinate care across the continuum with the care team and physicians.
• Engage with patient and family to ensure discharge goals are met and barriers are addressed.
