Job summary

Qualifications

• Experience as a staff nurse in an acute care hospital setting (minimum 3 years).
• Experience as a case manager in a hospital or payer-based model (minimum 2 years).
• Excellent written and verbal communication skills; ability to work under stress and collaborate with physicians and care teams.
• Licenses/Certifications: RN (Single State) at hire; Multi-state Compact RN preferred; Basic Life Support at hire; CCM preferred or Accredited CCM.

• Transitional care planning and care progression, with patient/family education.
• Collaborate with physicians and care team to assess needs, discharge goals, and barriers to discharge.
• Mentor new hires and participate in precepting as needed.
