Job summary

Wellstar Health System is seeking an RN Senior Case Manager to drive care progression, discharge planning, and complex care coordination within an acute hospital setting. You will partner with physicians, care teams, and families to optimize treatment plans and resource use.

Successful candidates will have strong communication skills, regulatory knowledge, and experience mentoring staff as part of a multidisciplinary team.

Qualifications

• 3+ years as staff nurse in acute care setting.
• 2+ years as hospital or payer-based case manager with care-team guidance.
• Strong knowledge of regulatory requirements and discharge planning processes.

Responsibilities

• Coordinate care progression and planning for discharge to meet patient needs.
• Collaborate with physicians, care teams, and family to optimize treatment plans.
• Mentor and precept new hires; participate in interdisciplinary rounds.
