Job summary

Dixon Social Interactive Services, Inc. is seeking a Care Management Extender in Greenville, NC to support Tailored Care Management teams with member engagement, coordination, and resource navigation.

The role enhances workforce capacity while following the Care Manager's direction and maintaining HIPAA compliance. The Extender helps members access services, understand care plans, and connect to community resources.

Qualifications

• At least 18 years of age.
• High school diploma or GED required.
• Experience in behavioral health, care coordination, or case management preferred.
• Knowledge of Medicaid, social services, or community resource navigation is a plus.
• Strong communication and organizational skills.
• Ability to work well in a team-based, fast-paced environment.
• Commitment to equity, inclusion, and trauma-informed care.

Responsibilities

• Assist with outreach and engagement activities to help members access services.
• Help members understand care plans and appointments.
• Provide education on community resources and Medicaid services.
• Help address barriers to care such as transportation, scheduling, or communication.
• Document interactions and activities in the care management system per policy.
• Communicate member needs and updates to the Care Manager.
• Support follow-up activities related to referrals and service linkage.
• Participate in team meetings as requested.
• Connect members to community-based services and supports.
• Provide guidance based on lived experience or community knowledge when applicable.
• Assist with social determinant of health needs (housing, food, employment resources).
• Work under the direction of the Care Manager and within delegated functions.
• Maintain confidentiality and HIPAA compliance.
• Ensure activities remain within the extender scope.
