Job summary

Cherokee Hospital is seeking a Care Management Extender for LTSS to support members in Tailored Care Management, I/DD/TBI, and CSFP programs. The role involves facilitating person-centered planning, linkage to services, and ongoing follow-up to improve client outcomes in a tribal health setting.

The extender will document in EHRs, collaborate with medical staff, and assist with discharge planning, transportation coordination, and community referrals while adhering to CIHA policies and tribal

Qualifications

• Care management knowledge of person-centered planning, referral/linkage, and follow-up.
• Ability to document information in EHR and care management platform.
• Strong communication with members, families, and care teams.
• Familiarity with care management processes and policies.

Responsibilities

• Provide care management functions per policy: planning, referral/linkage, monitoring and follow-up.
• Ensure care plan activities are carried out under supervision.
• Document medical, psychosocial, and other information in EHR.
• Engage members in person-centered planning to achieve outcomes.
• Coordinate with providers to facilitate discharge and follow-up.
