Job summary

P3 Health Partners is seeking a Provider Operations Coordinator in Omaha, NE. The role focuses on supporting HEDIS quality gap closures, risk adjustment activities, and provider engagement across affiliate clinics.

You will handle outreach, data mining, and scheduling while coordinating across multiple departments. The ideal candidate will have healthcare experience, strong organizational skills, and the ability to travel within the Omaha area.

Qualifications

• Experience in healthcare settings with patient data handling.
• Knowledge of CPT II codes preferred.
• CPR certification preferred.

Responsibilities

• Build relationships with Affiliate groups to achieve goals and serve as liaison across departments.
• Advise and support affiliate practices on quality gaps and documentation through outreach and data reviews.
• Coordinate patient outreach and scheduling to align with patient needs and availability.
• Partner with groups and departments to manage high/rising risk patients and coordinate care.
• Assist in CSR/IDT/JOC meeting preparation and participate as requested.
• Conduct data entry and maintain documentation for quality initiatives.
• Educate patients and affiliates on preventative screenings and their importance.
