P3 Health Partners, Inc. is seeking a Provider Operations Coordinator in Lincoln, NE. This on-site role supports HEDIS quality gap closures, risk adjustment activities, and provider engagement across affiliates.

The successful candidate will manage operational initiatives, coordinate education and outreach, and help ensure timely patient scheduling and care coordination with internal teams and providers.

Qualifications

• High school diploma/GED required; associate degree preferred.
• 2+ years in healthcare-related fields required; clinical health care or health plan experience preferred.
• Experience with Microsoft Word, Excel, PowerPoint, Outlook and general office equipment is required.
• Experience in Electronic Health Records (EHR) required.

Responsibilities

• Build and maintain relationships with Affiliate groups to achieve goals; serve as liaison between Affiliate groups and P3 departments.
• Support quality gaps (HEDIS/HOS) and conditional documentation via outreach, data mining/audits, and education of practice staff.
• Coordinate patient outreach and scheduling by finding appointment dates/times that meet patient needs.
• Partner with groups and departments to manage high/rising risk patients and ensure coordination of care across services.
• Assist in meeting preparation and participate in CSR/IDT/JOC meetings as requested.
• Enter data and maintain documentation for quality initiatives.
