Qualifications

• Requires strong interpersonal and communication abilities with patients and staff.
• Experience coordinating across multiple departments and affiliates.
• Healthcare experience with quality measures (HEDIS/HOS) preferred.
• Proficiency with EHR systems and office software is required.

Responsibilities

• Liaise between Affiliate groups and P3 departments to provide a single point of contact.
• Support HEDIS/HOS gaps through outreach, data mining, audits, and staff education.
• Prioritize patient outreach and schedule appointments to meet patient needs.
• Coordinate care across services for high/ rising risk patients.
• Prepare CSR/IDT/JOC meetings and participate as requested.
• Educate patients and affiliates on preventive screenings and follow-up.
