Job summary

Apicha Community Health Center in New York, NY, seeks a Care Coordination Programs Manager to oversee grant-funded programs in the Department of Support Services, including Care Coordination, Be Into Health, and PrEP/PEP Navigation. Qualifications

• BA/BS in relevant field with 5 years full-time experience; advanced degree considered with 3 years.
• At least 2 years of supervisory or leadership experience.
• Appreciation of and commitment to Apicha’s mission and values.
• Experience with target populations: low income, Serious Mental Illness, substance use disorders, and/or chronic illness.
• Experience with eCW, eSHARE, eICARE, Healthix, PSYKE preferred.
• Ownership, accountability, and humility across environments.
• Strong organizational skills and professional communication.

Responsibilities

• Ensure overall compliance of programs with service level, quality, budget, and reports.
• Assist the Director of Support Services with grants management including revenue projection and expense budget.
• Monitor staff records on enrollment, care plans, and related documentation.
• Monitor program deliverables and meet milestones.
• Operate within service quotas; adapt strategies to meet targets.
• Recruit and train program staff including supervision and performance reviews.
• Coordinate with other programs and departments for integrated services.
• Represent the agency at venues to promote services.
• Respond to inquiries from external entities such as Department of Health.
