Job summary

Apicha Community Health Center in New York, NY, is seeking a Medical Case Manager to provide direct case management for patients in our Primary Care Clinic, ensuring access to medical care and retention in treatment. The role involves coordinating services, authorizations, and reporting within a multidisciplinary team.

The ideal candidate will have a relevant degree or LPN with experience, strong English communication skills, and the ability to work evenings/weekends.

Qualifications

• Bachelor’s degree in social science, psychology, social work, health or equivalent experience or LPN with 1 year of relevant experience required.
• At least one year of case management experience preferred.
• Experience with communities served (HIV/AIDS, Asians and Pacific Islanders, immigrants, LGBT, families with children) preferred.
• Fluency in one Asian language preferred.
• Excellent written and verbal communication in English required.
• Independent and team-oriented with attention to detail.

Responsibilities

• Provide proactive case management to ensure access to medical care and services.
• Coordinate with PCP and multidisciplinary team for patient care.
• Obtain prior-authorization for services not covered by insurance.
• Maintain accurate EMR and paper records for reporting.
• Identify and recruit Asians and Pacific Islanders living with HIV/AIDS to support care.
• Collaborate with clinic teams for CQI activities.
• Perform other duties as assigned.
