Job summary

Apicha Community Health Center in Manhattan is seeking a Care Coordination Programs Manager to oversee grant-funded programs in the Department of Support Services, including Care Coordination, Be Into Health, and PrEP/PEP Navigation. The role leads program management, compliance, budgets, reporting, staff supervision, and quality improvement, coordinating with internal teams and external funders.

This on-site position requires a BA/BS with 5 years of experience (or advanced degree with 3 years),

Qualifications

BA/BS in a relevant field; 5 years full‑time experience required, advanced degree optional with 3 years' experience.
Minimum 2 years of supervisory or leadership experience with paid staff.
Experience with target populations and cultural competence in NY Chinatown area or NYC communities.
Experience with eCW/eSHARE/eICARE/Healthix/PSYKE preferred.

Responsibilities

Oversee grant-funded programs within the Department of Support Services, including Care Coordination, Be Into Health, and PrEP/PEP Navigation.
Ensure compliance with service, quality, budget, and reporting requirements; manage revenue projections and budgets.
Monitor records, deliverables, milestones, and program reporting; coordinate with internal/external stakeholders.
Recruit, train, supervise staff; perform performance reviews; provide coverage as needed.
