Job summary

Apicha Community Health Center is seeking a Care Coordination Team Leader to oversee psychosocial support for clients living with HIV and to lead the Be Into Health program at the 400 Broadway site. You will supervise staff, develop care plans, and coordinate services to enhance clients' well‑being and quality of life.

Qualifications

• Advance degree in mental health counselling or social work with two years of qualifying experience.
• Must hold a NY State license to provide mental health services (LMSW or equivalent).
• Experience supervising paid staff is a plus.
• Experience with target populations including individuals living with HIV/AIDS.
• Excellent MS Office skills; familiarity with AIRS, eSHARE and/or eCW preferred.
• Strong organizational and communication skills with professionalism.

Responsibilities

• Provide day-to-day oversight of Be Into Health program and implement services per policies.
• Develop and monitor program deliverables with the Care Coordination Programs Manager.
• Lead interdisciplinary client discussions and supervise the Care Coordinator.
• Prepare administrative reports and track travel and expenses.
• Conduct client care plans, counseling sessions, and group activities as needed.
• Ensure program records are accurate and compliant with protocols.
• Lead quality assurance activities and participate in CQI tasks.
• Collaborate with external agencies and internal staff to optimize care.
