Job summary

Virtua Health is seeking a part-time Care Coordination professional (RN or MSW) to partner with physicians and multidisciplinary teams to coordinate patient care across transitions and ensure high-quality, cost-effective outcomes.

The role requires 3+ years in clinical nursing or social work, plus 1 year in case management or homecare coordination within an acute care setting. RN or LSW is required; LCSW considered. On-site at Burlington Hospital with weekend coverage as needed.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum of 1 year Case Management or Homecare Coordination in an Acute Care setting; understanding of Medicare, Medicaid, managed care and discharge planning. (3-5 years preferred)
• RN or Masters prepared Social Worker; RN or LSW required; LCSW considered.

Responsibilities

• Coordinate patient care with physician and multidisciplinary team to enhance quality and cost-effective care.
• Complete assessments and plans; monitor length of stay; promote efficient utilization of resources.
• Maintain appropriate and complete documentation of plan of care in EMR and case management system.
• Participate in organizational improvement activities including patient satisfaction and compliance with regulations.
