Job summary

Virtua Health is seeking a dedicated RN/Case Manager to partner with physicians and a multidisciplinary team, coordinating patient care to improve quality and cost-effectiveness. You will advocate for patients and ensure safe transitions across the continuum.

Responsibilities include assessing needs, coordinating care plans, documenting in the EMR, and contributing to organizational improvement initiatives while complying with federal and state requirements.

Qualifications

• Minimum 3 years clinical nursing experience or social work.
• 1 year case management or interdisciplinary experience including homecare coordination in an acute care or behavioral health setting (3–5 years preferred).
• Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.

Responsibilities

• Care Coordination – plan and assess to meet patient needs; monitor length of stay and resource use.
• Maintain knowledge of clinical practice guidelines as a clinical expert.
• Collaborate with physicians to ensure seamless continuity of care.
• Document appropriate plan of care in EMR and case management systems.
• Contribute to organizational metrics, dashboards, and quality improvement activities.
• Refer appropriate cases for Social Work intervention and discharge planning guidance.
• Identify and document factors affecting patient progression; escalate as needed.
