The position requires an RN license or MSW, with 3+ years of clinical nursing or social work experience and 1 year in case management or homecare coordination.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum 1 year Case Management or Homecare Coordination in Acute Care (3-5 years preferred).
• Excellent verbal and written communication, problem solving, critical thinking.

Responsibilities

• Care Coordination – assess plans to meet patient needs and promote efficient resource use.
• Documentation – ensure complete plan of care in EMR and case management systems.
• Metrics – contribute to dashboards and organizational goals.
• Quality improvement – participate in Six Sigma and cross-functional teams.
• Compliance – identify and report regulatory issues as appropriate.
