Job summary

Virtua Health seeks a Nurse Case Manager / Social Worker to coordinate patient care and advocate for safe transitions across the care continuum. You will collaborate with physicians and a multidisciplinary team to optimize quality and cost-effective outcomes.

Responsibilities include care coordination, documentation in EMR, and participation in improvement activities. RN or MSW required; LCSW considered.

Qualifications

Minimum 3 years clinical nursing (RN) experience or Social Work.
Minimum 1 year Case Management or interdisciplinary experience including Homecare Coordination in an Acute Care setting; 3–5 years preferred.
Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.

Responsibilities

Care Coordination – assesses and plans to meet patient needs, monitors length of stay and resource utilization.
Acts as clinical expert and keeps knowledge of guidelines; coordinates with physicians for continuity of care.
Document of care plans in EMR and case management system; ensures complete records.
