Job summary

Virtua Health in Camden, NJ seeks a Care Coordination professional to partner with physicians and multidisciplinary teams to coordinate patient care, aiming to improve quality and cost‑effective outcomes. You will advocate for patients and facilitate safe transitions across the care continuum.

The role requires RN or Masters prepared Social Worker with 3+ years in clinical nursing or social work, and at least 1 year in case management or related settings, including Medicare/Medicaid knowledge.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work; 1 year case management or related interdisciplinary experience in acute care or behavioral health.
• Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.

Responsibilities

• Care Coordination – assess and plan to meet patient needs, monitor length of stay, promote efficient resource use, and document progress.
• Documentation – complete documentation of plan of care in EMR and case management systems.
• Metrics – accountable to goals and dashboards contributing to organizational success.
• Organizational improvement – participate in patient satisfaction, Six Sigma, and cross‑functional teams.
• Compliance – understand and apply federal/state requirements; identify and report issues.
