Job summary

Virtua Health in New Jersey is seeking a part-time RN or social work professional to coordinate patient care with a multidisciplinary team, aiming for high-quality, cost-effective outcomes. You will act as a resource and advocate to ensure safe transitions across the care continuum.

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum 1 year Case Management or other interdisciplinary experience including Homecare Coordination in an Acute Care setting or behavioral health in an inpatient outpatient or integrated behavioral health setting; understanding of Medicare, Medicaid, managed care and discharge planning. (3-5 years preferred)
• Excellent verbal and written communication skills, problem solving, critical thinking and conflict resolution.
• RN or LSW required; LCSW considered

Responsibilities

• Care Coordination - coordinates assessments and plans to meet patient needs, monitors length of stay and promotes efficient resource use.
• Documentation - completes documentation of the plan of care in EMR and case management systems.
• Metrics - accountable to goals and dashboards contributing to organizational success.
• Participates in organizational improvement activities including patient satisfaction, Six Sigma committees, department and divisional teams and community activities.
• Compliance - understands and applies federal and state requirements; identifies and reports compliance issues.
