Job summary

The Health Federation of Philadelphia is seeking a Health Services Navigator to support care coordination and improve patient outcomes. The role works closely with a clinical and administrative supervisor to coordinate records, referrals, and outreach for the ambulatory health services population.

The ideal candidate demonstrates empathy, confidentiality, and strong computer skills, with prior healthcare or social services experience preferred.

Qualifications

• High School Diploma or equivalent required.
• Some college or bachelor’s degree preferred.
• Must have excellent customer service skills.
• Prior experience in healthcare or social services preferred.
• Knowledge of Microsoft Office Suite and ability to learn new software.
• Prior experience with electronic health records preferred.
• Ability to interview patients and engage them in services.
• Ability to track activities and services provided.
• Knowledge of case management procedures preferred.

Responsibilities

• Obtain patient records from outside providers (e.g., discharge summaries, consult reports, diagnostic results).
• Follow up on patient referrals and diagnostic orders (e.g., track appointments and orders to completion).
• Provide care coordination services to at-risk patients (e.g., assist with referrals, schedule intake appointments).
• Perform outreach to patients due for care (e.g., screenings, overdue appointments).
• Provide patient education (e.g., how to make an appointment, how to contact after hours).
• Refer patients to internal and external resources as indicated.
• Track and document the outcome of activities in logs and provide monthly statistics.
• Monitor calls, voicemails, and inquiries and respond timely.
• Other duties as assigned.
