Job summary

Boston Medical Center, Corp. seeks a Recovery Support Navigator to provide care management and navigation for patients pursuing substance use treatment and primary care referrals. You will coordinate resources, assist with insurance navigation, and support treatment engagement in a busy, multispecialty setting.

The role emphasizes cultural and linguistic sensitivity, strong documentation, and collaboration with a multidisciplinary team to promote positive outcomes and access to services.

Qualifications

• Bachelor’s degree preferred in a related field.
• Degree in social work, psychology or related human services field highly preferred.
• Associate’s degree with healthcare experience is preferred.

Responsibilities

• Document in the electronic medical record and bill for services.
• Screen patients for substance use to determine level of care.
• Provide care management and navigation to engage patients in treatment.
• Coordinate referrals to primary care, withdrawal management and therapy.
• Monitor progress and follow up on visits and service needs.
• Educate, counsel, and support clients with culturally appropriate strategies.
• Develop relationships with community resources and collaborate with the care team.
• Document encounters, resources, and interventions with date/time in the EMR.
• Ensure compliance with policies, data collection and reporting requirements.
