Job summary

Mental Health Resource Center is seeking a System of Care (SOC) Navigator to coordinate internal and external referrals and promote timely access to integrated, person-centered services. The role focuses on uninsured and underinsured individuals and ensuring linkage within seven days of discharge.

The SOC Navigator engages clients before discharge, guides them through MHRC’s CSC Central Receiving System, and works to prevent crises by proactive outreach and coordination with multi-disciplinary

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Minimum 1 year of experience in human services or behavioral health.
• Experience working with adults with mental illness preferred.
• Strong communication and problem-solving skills required.
• Ability to work independently and manage multiple referrals.

Responsibilities

• Care coordination and referral management for MHRC programs.
• Discharge planning and follow-up within seven days of discharge.
• Advocacy and community engagement with local partners and providers.
• Documentation and compliance in EHR with timely updates.
• Maintain knowledge of MHRC programs, intake pathways, and resources.
