Job summary

Delta View Post Acute in Antioch, CA is seeking a dedicated Kaiser Liaison – LVN for a full-time role. You will coordinate admissions, transfers, and discharges, acting as the primary communication link between Kaiser Permanente and the facility to ensure seamless resident transitions and adherence to care plans.

The ideal candidate holds a California LVN license, has strong communication and organizational skills, and can collaborate with a multidisciplinary team to support quality resident

Qualifications

• Active LVN license in California.
• Experience coordinating care with Kaiser Permanente or similar teams is preferred.
• Strong communication and organizational abilities with attention to detail.

Responsibilities
