Job summary

Community Care Network seeks a Manager of Care Coordination to lead planning, administration, and oversight of care coordination services, including HCBS and Bridge Care. You will supervise Care Coordinators, ensure compliance with regulations, and promote quality outcomes for individuals served in a transitioning, person-centered system.

Qualifications include a Bachelor's or equivalent and a QDDP certification, plus 4+ years in developmental services and leadership experience.

Qualifications

• Bachelor’s degree in a related field preferred; equivalent experience may be considered.
• QDDP certification required.
• Leadership or management experience required.
• Experience with EMR data entry and record keeping.
• Strong writing and organizational skills for efficient communication.
• Must meet background check requirements.

Responsibilities

• Lead planning, development, and oversight of HCBS and Bridge Care care coordination services.
• Ensure quality of life and rights of individuals served.
• Support transition to conflict-free case management; collaborate with CMOs.
• Build strong relationships with individuals, families, guardians, providers, and agencies.
• Monitor coordination between providers, CMOs, and interdisciplinary teams.
• Ensure compliance with regulations and HCBS requirements.
• Coach and supervise Care Coordinators; provide feedback and development.
• Recruitment and onboarding of new staff and providers as needed.
• Attend trainings, meetings, and supervision sessions to stay current.
