Qualifications

• Minimum of 4 years of experience in developmental services or care coordination.
• Previous leadership or management experience required.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Lead and oversee care coordination services for HCBS and the Bridge Care Program.
• Monitor compliance with regulatory requirements.
• Support transitions in services and ensure quality of life for individuals.
