Job summary

Rising Star Home Care Services in Edgewood, MD seeks a Care Coordinator (Full Time) to build and manage the master schedule, ensuring each client has the right caregiver for every authorized hour. You will coordinate with families and the nursing team to deliver consistent, quality in-home care.

The role requires an active CNA or GNA, at least one year of home care experience, strong organizational skills, and proficiency with scheduling software and Maryland LTSS.

Qualifications

• Active CNA or GNA certification.
• At least one year of hands-on experience in home care, home health, assisted living, or senior care.
• A process mindset with checklists, clean files, and schedules that work.
• Strong follow-through; when you say it's handled, it is.
• Comfort with scheduling software and electronic visit verification (Maryland LTSS experience is a big plus).
• Warm, clear communication with caregivers, clients, and families.
• Valid driver’s license and reliable vehicle; willingness to travel to client homes.
• Ability to pass a criminal background check.

Responsibilities

• Build and manage the master schedule so every client has the right caregiver for every authorized hour.
• Handle call outs and fill open shifts quickly so no client goes without care.
• Visit client homes to introduce new caregivers and check in with families.
• Screen caregiver applicants and ensure files are complete before start.
• Correct missed clock-ins/outs in Maryland LTSS the same day flagged.
• Track caregiver CPR, training, and certifications; follow up before expiry.
• Help coordinate client assessments with the nursing team.
• Document processes to improve operations.
