Qualifications

Responsibilities

• Supervise and coordinate activities of Referral Specialists.
• Analyze management reports and workforce metrics.
• Ensure timely and accurate linkage to care coordination.

Skills

Advanced communication skills

Advanced knowledge in Microsoft Office Suite

Knowledge of community health systems

Ability to manage deadlines

Education

High school degree or GED

Bachelor’s degree in healthcare or behavioral health
