Qualifications

• Knowledge of discharge planning, utilization management, and case management.

Responsibilities

• Leads self/others/leaders as per OneCHRISTUS competencies.
• Interviews patients/families to assess social, emotional, and financial factors.
• Develops and monitors the plan of care with CMII/CMIII.
• Coordinates care progression and efficient discharge planning.
• Communicates with care team and documents in the medical record.
