Job summary

CHRISTUS Health seeks a Per Diem Care Manager (CM) to collaborate with patients, families, physicians, and a multidisciplinary team to ensure seamless progression through the care continuum. The CM identifies barriers, coordinates care, and supports safe, timely discharges across settings.

The role emphasizes comprehensive assessment, planning, and evaluation of needs with a focus on patient safety, satisfaction, and resource optimization. PRN schedule fosters flexible coverage.

Qualifications

• Excellent verbal and written communication, ability to interact with diverse populations
• Critical and analytical thinking skills
• Working knowledge of discharge planning, utilization management, case management, and managed care reimbursement

Responsibilities

• Interviews patients/families to assess social, emotional, and financial factors affecting health and discharge planning
• Develops and monitors the patient’s plan of care with CMII/CMIII for effectiveness
• Coordinates patient care progression throughout the continuum of care to optimize throughput and reduce delays
• Advocates for patient needs and resources, including post-acute placement options
• Collaborates with medical staff to remove barriers and ensure appropriate use of resources
• Monitors length of stay and communicates with team to expedite discharge
• Documents in the medical record per regulatory guidelines
• Participates in Multidisciplinary/Patient Care Progression Rounds
• Alerts Physician Advisors/CM Director to escalated cases
