Caring for Others in Medford, OR is seeking a dedicated care coordination professional to serve as the central hub for Clients, families, Caregivers, and partners. You will manage care coordination, scheduling, client assessments, compliance, and branch operations to ensure high-quality, compliant care.

You'll lead training, maintain licensing docs, and support billing accuracy while delivering 5-star service and building strong collaborative relationships with the team and community.

Qualifikationen

• Experience in home care, healthcare coordination, or a client-facing service role preferred.
• Highly organized with strong attention to detail and accurate record-keeping practices.
• Proactive communicator with excellent interpersonal skills and a warm, professional presence.
• Solutions-oriented and self-managed — able to handle multiple priorities in a fast-paced environment.
• Committed to compliance, HIPAA standards, and maintaining accurate documentation.

Aufgaben

• Be the hub for care coordination, compliance, scheduling, Client communication, and Branch operations.
• Deliver 5-star service on every call as the communication hub for Clients, families, Caregivers, Case Managers, and referrals.
• Coordinate care services for timely, consistent, appropriate delivery to all Clients in home and by phone.
• Develop individualized service plans and conduct initial and ongoing Client assessments.
• Manage authorizations and balancing activities to support billing accuracy.
• Support staffing to minimize overtime, ensure consistent coverage of all shifts, and maintain licensing documentation.
• Lead weekly Caregiver New Hire training and prepare intake materials.
