Job summary

Memorial Home Services seeks a dedicated care coordination professional to support multidisciplinary teams in planning and delivering patient-centered care. You will ensure accurate documentation, facilitate discharge planning, and coordinate services with caregivers and external agencies.

The role requires a Bachelor's degree in a relevant field and active engagement in rounds, CPR/BLS training, and ongoing professional development.

Qualifications

• Bachelor's degree in human services, psychology, sociology, social work, or related healthcare field required.
• Maintains appropriate documentation and coordinates discharge planning with caregivers.
• Assesses patient needs and develops discharge plans and goals of care.
• Provides information to social service/discharge planners to facilitate discharge.
• Participates in multidisciplinary rounds and care conferences.

Responsibilities

• On-call as scheduled and responds as needed.
• Upgrades professional skills and completes reports.
• Assists with referrals to related agencies considering freedom of choice.
• Acts as liaison with nursing, medical staff and community agencies to coordinate services.
• Demonstrates clinical competency via CPR/BLS modules.
