Amedisys is seeking an RN Case Manager in Claremore, OK to guide patients and families through the care continuum. Under RN/MD supervision, you will assess needs, coordinate care, support discharge planning and manage outpatient care, while educating and coaching the care team.

You will collaborate with a Care Advocate and Social Worker to ensure the appropriate level of care, and contribute to ongoing patient outcomes in a supportive environment with opportunities for professional growth.

Qualifications

• Registered nurses with current state license and CPR certification.
• Experience coordinating patient care plans and discharge planning.
• Ability to educate, coach, and support patients and families.

Responsabilités

• Perform patient assessments and collaborate to develop a care plan.
• Refer to other disciplines as needed and document rationale.
• Promote health and independence through patient education and rehab.
• Supervise LPNs and HHAs as assigned.
• Complete timely and accurate documentation.
• Communicate patient progress to clinical management and team.
• Plan and provide staff education.
• Fulfill on-call responsibilities and services.
• Engage in clinical development and ongoing education.
