Job summary

AMN Healthcare seeks a Case Manager to join our multidisciplinary care management team in Oklahoma City. You will coordinate care, assess patient goals, and monitor progress to ensure appropriate treatment and discharge planning.

Required NB: Oklahoma RN license or eNLC, BLS, 3 years acute care, and strong communication skills. You will work 12-hour day shifts in a high-collaboration environment with focus on patient advocacy and resource coordination.

Qualifications

• RN license in Oklahoma or eNLC multi-state compact.
• BLS certification from American Heart Association.
• Minimum 3 years of clinical nursing in acute care.
• Knowledge of nursing practices and procedures.
• Strong clinical assessment and communication skills.
• Familiarity with community resources and third-party payer issues.
• Ability to explain complex information clearly to patients and families.

Responsibilities

• Coordinate care for selected patient populations within a multidisciplinary team.
• Assess patient goals including biophysical, psychosocial, and discharge needs.
• Monitor progress and adjust care plans as needed.
• Advocate for patients and families with scarce resources.
• Collaborate with third-party payers to resolve care continuity issues.
• Direct discharge planning with the care team, family, and community.
