Job summary

Qualifications

Responsibilities

• Plan patient care with physicians and health-care team.
• Ensure care/discharge plans align with clinical course and patient needs.
• Identify barriers to discharge and coordinate timely transitions.
• Participate in multidisciplinary rounds and discharge planning.
• Arrange post-acute care settings and payer authorization where needed.
• Track readmission reasons and analyze discharge-related trends.
