Job summary

INTEGRIS Baptist Medical Center in Oklahoma City, OK is seeking a full-time RN Case Manager for the day shift with every-other-weekend coverage. You will assess patient needs and coordinate care across clinical teams to ensure appropriate discharge planning.

Required include 2 years of clinical experience and a current Oklahoma RN license with BLS. Preferred are a BSN and Case Management Certification, plus strong communication and computer skills to navigate CMS/payor guidelines.

Qualifications

• 2 years experience in a clinical setting (e.g., home health, inpatient, physician office, clinic).
• BLS issued by American Red Cross or American Heart Association within 30 days of hire.
• RN license current in Oklahoma or multi-state license via eNLC.

Responsibilities

• Completes a comprehensive assessment of patients' clinical, psychological and financial needs using all resources.
• Coordinates timely transfers to appropriate levels of care based on clinical needs and criteria.
• Develops and updates discharge plans including referrals to healthcare and community resources.
