Job summary

A Caring Alternative, LLC is a CARF-accredited service provider delivering behavioral health and substance use disorder services across Western North Carolina, including Morganton. We seek a Tailored Care Management (TCM) Care Manager to coordinate medical, behavioral health, and social services to improve client outcomes.

The role requires a high school diploma and at least one year of paid experience with the TCM eligible population, plus a valid driver's license and reliable transportation.

Qualifications

• High School Diploma or equivalent required.
• At least one year paid experience with the TCM eligible population.
• Ability to use Microsoft 365, Excel and Word.
• Valid driver’s license, auto insurance, and reliable transportation.
• Pass criminal background and regulatory checks.

Responsibilities

• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments.
• Engage in health promotion activities.
• Provide and track referrals and assist in obtaining community resources and social support services.
• Participate in case conferences.
• Support the care manager in assessing unmet health-related resource needs.
