Qualifications

• At least one year of paid experience working directly with the TCM eligible population.
• Proficiency with Microsoft 365 (Excel, Word, etc.).

Responsibilities

• Engage with members, perform outreach and follow-up.
• Coordinate services and appointments.
• Assist in health promotion activities and link clients to resources.
